Care Coordination: Empowering Families

Care Coordination: Empowering FamiliesWhen it comes to getting medical care for their child, the role of the parent or primary caregiver is an important one. Often the parent has to work with many different people in addition to doctors.

The purpose of the Care Coordination: Empowering Families training is to help parents learn:

  • How to identify a medical home
  • Techniques to organize information and find reliable resources
  • How to navigate health care and insurance systems
  • Skills to increase communication and coordination between multiple providers
  • How to plan early for child’s transition
  • Ideas for coping with stress and finding support
  • Advocacy skills

Region 4 is seeking partners interested in working together to provide the training to other parents. For more information, please contact the Region 4 Parent Coordinator, Kristen Hawkins at khawkins@mphi.org
